Begin at Islip Pharmacy and Surgical, heading south before turning left onto E Main Street. Continue for 0.4 miles, then turn left onto Islip Avenue and drive 1.8 miles. Merge onto the Southern State Parkway heading west toward New York. Follow the parkway for about 3.7 miles, then take exit 41A to Sagtikos Parkway north. Continue 4 miles and take the ramp for I-495 West toward New York. Stay on I-495 West for 12.5 miles.

Take exit 42 to merge onto Northern State Parkway westbound. After 2.5 miles, take exit 33 for Wantagh Parkway South, then exit W2 W for Old Country Road toward Westbury. Turn right onto Old Country Road and continue for 0.4 miles. Finally, turn right onto State Street, and Elder Care Homecare Long Island will be on your right at 1025 Old Country Rd #314.
